While many families are familiar with traditional hospital births, more mothers are exploring options that offer a more personalized, family-centered approach to pregnancy, labor, birth, and postpartum care.
This month, San Antonio Super Moms is honored to introduce Heather Hilton Johnson , founder of Birth Center of San Antonio , and share the story and vision behind a local birth center dedicated to providing families with individualized midwifery care in a calm, supportive environment.
A mother of four, wife, and experienced midwife, Heather began her midwifery journey in 2008 and has spent years serving families in both home birth and birth center settings. Her passion for creating positive birth experiences ultimately inspired her vision for Birth Center of San Antonio — a space designed to feel like home while providing mothers and families with personalized care throughout their pregnancy and birth journey.
Birth Center of San Antonio offers families access to a variety of maternity and midwifery services, including prenatal care, birth center births, water birth, home birth, VBAC support for eligible mothers, and postpartum care. At the heart of the practice is an emphasis on building meaningful relationships with families and helping expecting parents feel informed, supported, and confident in their care.
As San Antonio continues to grow, Heather believes there is an increasing need for pregnancy care providers and greater access to midwifery care for families seeking alternatives to a traditional hospital birth experience.
We had the opportunity to speak with Heather about her journey into midwifery, what inspired her to create Birth Center of San Antonio, the experience of giving birth in a birth center, and her hopes for helping shape a more mother-and-baby-centered birth culture in San Antonio.
Meet Heather Hilton Johnson, Founder of Birth Center of San Antonio
Heather, can you tell us a little about yourself and what inspired you to open Birth Center of San Antonio?
I am a mother of four amazing children, a wife, and a midwife. I started my midwifery journey in 2008 and have had the opportunity to serve families in both home birth and birth center settings throughout the years.
While home birth has a special place in my heart, I feel that birth centers are able to care for a greater number of people at such an important time in their lives. I also really enjoy working as part of a team, and the birth center model allows for both!

What was your vision when creating Birth Center of San Antonio, and what gap did you hope to fill for local families?
My vision was to create a birth center that feels like home and gives moms the individualized care that is so often lacking in modern obstetric care.
Our city is big and growing! We need more pregnancy care providers, and especially more midwives.
Many expecting parents are familiar with hospital births but may not know much about birth centers. How would you describe the birth center experience to someone hearing about it for the first time?
The birth center experience is very personalized, and each experience will be different. However, you can expect a low-intervention birth in a calm environment, while being cared for by the team you’ve spent so many months getting to know.
You can also expect uninterrupted bonding time with your baby, gentle hands available to care for you and your baby when needed, and access to nearby higher-level hospital care when necessary.
Your philosophy focuses on personalized, family-centered care. What does that look like throughout pregnancy, labor, birth, and postpartum recovery?
Personalized care means recognizing that some people need more emotional support, some need more nutritional support, and others need more education or resources to help them make decisions about their care. It also means that you have choices!
We very much see pregnancy and birth as a whole-family event, so siblings are welcome to be part of the experience, as are grandparents, aunties, and anyone else who makes you feel supported.
Birth Center of San Antonio offers services such as water birth, home birth, VBAC support, and postpartum care. What are some of the benefits families appreciate most about these options?
Hydrotherapy and water birth are wonderful tools for pain relief, as well as offering many other benefits.
Home birth may be a great option for those who live near the birth center but don’t want to leave their home while in labor. You receive the same amazing team and level of care in either setting.
VBAC is an option for some mothers who have had a previous cesarean and wish to try for a vaginal birth. Having a supportive provider vastly increases VBAC success rates.
Families in our care will have four regularly scheduled postpartum care visits during the first six weeks for both mom and baby. This allows us to truly nurture the postpartum mother, help with breastfeeding, and support her in navigating emotional challenges.
We also provide care for the baby, including breastfeeding evaluation, weight checks, jaundice checks, newborn screening, hearing screening, and congenital heart screening. For many of our families, their first pediatrician visit is around eight weeks postpartum.
One thing that stands out about your practice is the emphasis on building relationships with families. Why is that relationship so important during pregnancy and birth?
It’s important to have trust and a good rapport with whoever is in your birth space. Because our care is individualized, we really get to know the families we serve. This makes for a better overall birth experience!
What does a typical prenatal visit at Birth Center of San Antonio look like, and how does it differ from a traditional medical setting?
Prenatal visits are monthly until 28 weeks, then every two weeks until 36 weeks, and weekly from 36 weeks until delivery. Our visits are scheduled to be 45 minutes long.
A typical prenatal visit includes gathering clinical information such as blood pressure, pulse, weight, and urinalysis; drawing labs; assessing symptoms; providing referrals for a higher level of care when needed; and conducting a general risk assessment.
We also talk about things like nutrition, how someone is doing emotionally, any general questions they might have, how they are sleeping, what resources they may need at that point in their pregnancy or life, and what supplements and natural methods are safe.
Our care is truly personalized for each family, so a variety of topics may be covered during each visit.
Pregnancy and birth can feel overwhelming, especially for first-time parents. How do you help mothers and their partners feel confident, informed, and prepared for the journey ahead?
By getting to know them, understanding what their concerns are and what their hopes are, and making sure they are well-educated, informed, and given the resources they need to feel confident.
Looking back on your career, what has been one of the most meaningful birth stories or moments that reaffirmed why you chose this path?
There have been hundreds of moments throughout this journey that have reaffirmed this path for me. I absolutely love what I do, and I’m so passionate about creating positive birth experiences.
The specific moments that stand out for me have been the births that were very challenging and required a lot of trust and perseverance. To see moms overcome challenges and struggles, put all their trust and faith in their bodies, their babies, and their birth team, and then come out on the other side victorious is just so special.
If a mother is considering a birth center but isn’t sure whether it’s the right fit for her, what would you want her to know?
I would encourage her to reach out and schedule a consultation with the midwife. We can chat with you and see if you’re a good candidate for birth with us.
Birth Center of San Antonio recently opened its doors to local families. What are your hopes and goals for the future of the birth center and the families you serve?
We hope to reach a larger community of families who are looking for a more personalized, low-intervention style of care but also want the comfort of being near a hospital in case there is a need for a higher level of care.
Ultimately, we hope to help change the birth culture here in San Antonio to one that is more mother-and-baby-centered.
